Today is the day to start a conversation with your family and whānau about what matters to you for your future healthcare.

Advance care planning gives everyone, including you, a chance to think about what’s important, and what to say, to your healthcare team for your end-of-life care.

Advance care planning can be done one step at a time, at your own pace.  The important steps are thinking about and talking about your wishes, writing them down and sharing them.

Ask your GP, practice nurse or healthcare team for a copy of an advance care planning guide.
