What matters most for your future care?
Today is the day to start a conversation with your family and whānau about what matters to you for your future healthcare.
Advance care planning gives everyone, including you, a chance to think about what’s important, and what to say, to your healthcare team for your end-of-life care.
Advance care planning can be done one step at a time, at your own pace. The important steps are thinking about and talking about your wishes, writing them down and sharing them.
Ask your GP, practice nurse or healthcare team for a copy of an advance care planning guide.

When you have an Advance Care Plan everyone around you knows what is important to you and what treatment and care you would like.
