Nelson - Mobile Community Team (MCT)

Assessment, intervention and ongoing treatment for people over the age of 18 with serious psychiatric disorder or severe emotional and behavioural problems of a psychological nature.

Referral Criteria
The Mobile Community Team will accept referrals from general practitioners, self, hospital, community agencies, and family or friends with client’s permission.

Nelson – Continuing Care Team (CCT)

Continuing care case management provides ongoing support for those living with a significant, chronic mental health disorder.

Continuing care case managers are registered health professionals who provide individualized support to clients and their families.

Case management may include ongoing education about mental illness, support with medication, attending appointments, maintaining relationships with family, accommodation providers and employers, coordination of involved services  and general oversight to support recovery.

Referral Criteria
CCT works with people with enduring mental illness over the age of 18. Referral to CCT case management is via internal transfer or via CARES.

Nelson - Kawai Clinic

Kawai Clinic provides quality outpatient assessment and treatment for adults with mental health problems.

Referral criteria

  • any health professional can make a referral by letter or fax or phone (followed by a letter)
  • people can self refer although we prefer your GP to be involved in the referral.
  • referrals are screened for appropriateness and urgency.
  • assessment appointments are made as soon as possible. We currently operate with a waiting list.
  • the Kawai Clinic does not provide relationship counselling, anger management, treatment for alcohol and drug issues or for sexual abuse issues that qualify for ACC funding.

Our hours are from 8.30am to 5pm, Monday to Friday, excluding public holidays.

Nelson - Home Based Treatment Team (HBT)

HBT is an intensive treatment and support service for people experiencing acute mental health problems as an alternative to hospital based care. Priority is given to those people who would otherwise require acute hospital based care.

Witherlea House

Witherlea House provides psychiatric assessment and treatment, psychological assessment and treatment, case management within a multi-disciplinary framework for people experiencing severe and enduring mental health problems, Forensic and court liaison and primary care and general practitioner liaison.

The Community Mental Health Service will accept referrals from general practitioners, self, hospital, community agencies, and family or friends with client’s permission who meet the criteria.  Phone enquiries are always welcome to help referrers establish the relevance or usefulness of services available for their client.

Nelson Marlborough - Needs Assessment & Service Coordination (NASC)

The Need Assessment & Service Coordination (NASC) service provides specialist assessment for persons with high and complex needs, service planning and coordination and liaison support with local, regional and national service providers

A Needs Assessment is a tool to help find out what effect a mental illness has on a person’s life and helps identify the abilities, resources, goals, needs and the supports which would be useful to themselves and their family and whanau.

Needs Assessments are done to put appropriate supports in place. Copies are usually held by those people and agencies who support you. We ask you to approve the release of this information to these people/agencies. If they do not have this information, they may not be able to properly support you.

You have the right to request a different person to complete your Need Assessment, or to have an assessment by the Kaupapa Maori Need Assessment Service.

Nelson Marlborough - Oranga Toi Ora Maori Mental Health

Oranga Toi Ora provides specialist crisis and routine assessment, care management and consultancy to assist clients to move toward holistic wellness.

We foster and seek to protect Maori life – principles by providing mental health services in which Maori values and health are inherent.

The principles of equity, accessibility, freedom of choice and the right to quality are sustained in a culturally sensitive service that is appropriately either Maori, mainstream or a combination of both.

Nelson Marlborough: Older Persons' Mental Health

Nelson Marlborough: Older Persons Mental Health

The Older Persons Mental Health (OPMH) team provide a specialist services that offers consultation and assessment; and coordinates care for older adults living with significant mental illness and/or cognitive decline and associated challenging behaviours alongside age related physical health issues.

Referral Criteria

Older adults with:

  • Significant mental health problems

Adults of any age with:

  • An established diagnosis of dementia (Alzheimer’s, vascular, mixed or idiopathic front temporal dementia type) complicated by significant behavioural or psychological symptoms

A person 65 years and older will generally be better served by receiving their services from general adult mental health service providers if:

  • they do not have significant age-related conditions of any kind
  • they have an existing relationship with general adult mental health service providers
  • they affiliate culturally and socially with adults under the age of 65. 

Uncomplicated dementia and early dementia without significant mental health disturbances will be referred to gerontology in the first instance.

Referrals to the OPMH team need to be completed by your GP. If unsure regarding referral, please contact the team.

Rural Community Mental Health Service

A 24 hour, 7 day a week service for people experiencing a mental health crisis which includes:

  • Assisting the local GPs’s in responding to psychiatric crises
  • linking clients into other mental health services
  • supporting the client and their families and whanau

The service also offers case management for mental health consumers in Golden Bay and includes:

  • follow up in the community
  • wellness maintenance and recovery planning to assist in the management of a mental illness
  • assistance in recovering from the effects of having a mental illness
