He pukenga hauora
He Pukenga Hauora refers to the service created to support the cultural needs of whanau admitted to either Nelson or Wairau hospitals that identify as Maori.
- Supports whanau with information that aids understanding of hospital process, procedures and expectations.
- Provides whanau with information that facilitates active participation in the treatment and discharge planning process. This may include facilitation of whanau hui to enhance understanding of proposed care and treatment options.
- Offers advocacy and referral on discharge to a range of community services.
He Pukenga Hauora will visit you on the ward to discuss what supports are available so you can decide whether to accept service support.
Services are available Monday to Friday, 8am-4.30pm.
